We welcome your participation in the Dysautonomia Foundation’s 2006 Journal, which commemorates the year’s activities and provides you with an opportunity to recognize and remember those who are very special to you and to the Foundation.

For repeat pages from a prior year, please indicate the year and page number, if possible.
Please mail the form and your check no later than December 26th, 2006. If you prefer to pay online using a credit card, please use the General Donation option on our online donation page and be sure to include the word “Journal” in the reference field.
If your submission is handwritten or typed, we will typeset it for you. Artwork must be camera ready. If you want to include a business card in your submission, please do not use staples or tape on the face of business cards. We can also include printed photographs and photos on disks.
