Dear Friends: We welcome your participation in the Dysautonomia Foundation’s 2007 Journal Campaign, which commemorates the year’s activities and provides you with an opportunity to recognize and remember those who are very special to you and to the Foundation. Proceeds from this campaign will be used for clinical care and research to benefit people with FD. Through your kindness and generosity with past contributions, we have been able to provide the highest level of medical care and scientific research available to the FD community. This year, we have undertaken an expansion of the FD treatment center at NYU Medical Center, including the addition of a clinical research laboratory and the endowment of a second professorship dedicated exclusively to research and treatment of patients with dysautonomia. You can submit your form by mail (to: Dysautonomia Foundation, 315 W. 39th Street - Suite 701, New York, NY 10018), by fax (to: 212-279-2066) or by email (to: info@famdys.org) You can pay for your submission by mail (enclose a check with your form, mailed to: Dysautonomia Foundation, 315 W. 39th Street - Suite 701, New York, NY 10018), by credit card (call us at: 212-279-1066 or provide your credit card online. Journal submissions can be text only, or can include black and white photos and camera-ready artwork. Color artwork and photos can only be accepted for Diamond and Jewel pages, you can submit black & white or grayscale artwork for all other pages. Artwork and photos can be provided in a PDF, JPG, BMP, or Word file via email to info@famdys.org or by mailing a CD to our office. If you participated in our Journal campaign last year and would like to repeat an identical submission in this year’s Journal, please be sure to indicate on the form or in your email that this is a repeat submission, including the relevant page number from last year's Journal. Please provide your submission by December 31st, 2007 to allow us to produce and distribute the Journal in timely manner. Unfortunately, distribution of last year’s Journal was delayed due to a printing error on the submission form; we apologize for the delay and assure you that this year’s Journal will be mailed out in early spring.
